Checkboxes in Form to Check Checkboxes on PDF Document

Currently we have the system set to require a Checkbox be checked for the Merge to Check a Checkbox in a Document.  We are planning on adding more conditionality to allow other form elements to Check a Checkbox, but currently only the Checkbox function in the forms will do.  Below are some simple screenshots:

 

This is a simple form setup where the name section that pops up conditionally also pops up with a checkbox underneath it asking the person filling out the form to confirm the spelling of the name.

 

Then the template inside of the merge is edited to check the proper checkbox in the PDF document every time the trigger checkbox in the form is checked.  The name and the checkbox are set to show up conditionally based on the selector right before them in the form.  The checkbox is made a required check when it shows up conditionally.

 

Below is the set of conditions set to show the two different options of Original Judgment Creditor or New Assignee of Record.  There are “opposite actions” checked in the conditional rule in the conditions section of the form settings that show/hide and show/hide the opposite set of names and opposite checkbox that correspond to the OJC or the Assignee.

 

Here is the Merge setting with the 2 and the 3 selected that indicate the 2nd or the 3rd button for the same PDF Document used in the Template.
